The first time an owner tells me their dog “can’t handle car rides,” I ask them to describe exactly what happens. Drooling before the car even starts moving. Panting hard by the first stop sign. Vomiting only on long trips, or every single time, even a five-minute drive to the vet. The answer changes what I recommend, because ginger and chamomile are not two flavors of the same fix — they work on different parts of the problem, and conflating them is where a lot of well-meaning advice goes wrong.
The physiology owners are actually fighting
Car sickness in dogs is largely a vestibular problem: the inner ear senses motion that the eyes and body aren’t confirming, and the resulting sensory mismatch triggers the vomiting center in the brainstem. In young puppies this is often purely mechanical — their vestibular system hasn’t matured, and most outgrow it by around a year. In adult dogs, a second layer gets added: dogs that had a bad early experience with the car (often their first ride, to be neutered or vaccinated) can develop anticipatory anxiety that makes the nausea worse and lingers even after the vestibular sensitivity would otherwise fade. That’s the distinction the panel keeps coming back to. Ginger addresses the nausea signal. Chamomile addresses the anxiety signal. A dog with true motion sickness and no fear component may do fine on ginger alone. A dog that’s anxious but not actually nauseated may do fine on chamomile alone. That mechanism was demonstrated directly on human and native enteric-neuron receptor tissue in a study published in Neurogastroenterology & Motility, which found ginger’s constituents non-competitively inhibit 5-HT3 receptor activation. A review in Nature Reviews Gastroenterology & Hepatology lays out the broader picture: ginger acts peripherally in the GI tract, increasing gastric tone and motility through anticholinergic and antiserotonergic action, which helps explain why it’s historically been used for motion sickness, pregnancy-related nausea, and chemotherapy-induced nausea across species. Closer to the dog-specific mechanism, a study in PMC found that ginger’s 6-shogaol attenuates activation of gastroesophageal vagal afferent C-fibers — the nerve pathway that reports gut irritation up to the brainstem’s vomiting center — in a mouse model. That’s mechanistic evidence, not a canine car-sickness trial, and the panel is careful about that gap: strong biological plausibility and cross-species mechanism data, but the well-controlled, dog-specific, on-road RCT that would settle dosing and effect size for car sickness specifically doesn’t yet exist in the literature I can point an owner to. That’s a thin spot, not a disqualifying one — plenty of what veterinary medicine currently uses, on the same evidence tier, gets prescribed with a label written for a different species. I’d rather say that plainly than pretend the citation is more direct than it is.
One of the dogs I worked with was a nine-year-old, 62-pound Labrador who’d been fine in the car her whole life until a long highway trip in July left her drooling and vomiting twice in ninety minutes. The owner, understandably rattled, had already tried a chamomile-based calming chew from the pet store with no change. When we walked through the history, there was no anxiety at all — she jumped into the car eagerly, tail up, no panting before the engine started. The vomiting only ever showed up on trips over about forty minutes, always the same way: quiet for the first stretch, then drooling, then sick. That pattern reads as motion sickness without a fear component, which is a case where I’d expect an antiemetic mechanism to matter more than a sedative one, and it’s the kind of history that should change what you reach for before you change the dose.
Chamomile: real, but for a different job
Chamomile’s reputation as “calming” rests on a genuine and well-characterized mechanism: apigenin, one of its main flavonoids, binds to GABA-A receptors, the same inhibitory receptor system that benzodiazepines act on, though far more mildly. That’s a real anxiolytic pathway, documented in the NCCIH fact sheet on chamomile’s evidence base, which also notes that most controlled human trial data covers general anxiety and sleep — not motion sickness, and not dogs specifically. What chamomile is not, on the current evidence, is an antiemetic. It doesn’t touch 5-HT3 receptors the way ginger does, and it isn’t shown to change gastric motility. Where it earns its place on the panel’s shortlist is in the anxious-anticipation cases: the dog that starts panting and pacing the moment the leash comes out and the car keys jingle, before the car has moved an inch. For that dog, a sedating, GABA-active ingredient is doing the job the situation actually calls for. Giving that dog ginger alone treats a stomach that may not be the problem.

Reading the label and the history before the claim
I tell owners the same thing every time this comes up: read the label and the bloodwork before you believe the claim on the box. A “calming and car-sickness” chew that lists chamomile as the lead ingredient and ginger as a trace amount further down the list is, functionally, a calming product wearing a car-sickness label. That’s not necessarily dishonest, but it’s not going to do much for a dog whose problem is purely vestibular. Check the actual order and amount of active ingredients, not just the claim on the front panel, and match it to what your dog’s history actually points to — true nausea, true anxiety, or, in a fair number of dogs I see, both, which is the case where a combination product or a two-ingredient approach under veterinary guidance makes the most sense.
